Before you call have the following available

1. Insurance Card – needed to schedule your appointment.
2. Primary Physician – the name of your primary physician

We will call a few days prior to your appointment to confirm your appointment. If we cannot reach you we leave a message. Please call back to confirm your scheduled appointment. (617) 731-1600, option 1, option 2.

Your Office Visit

What to bring for your appointment:

For Medical or Cosmetic Appointments:

• Insurance card
• Patient Information and Health History Forms
• Physician referral - If you do not have a referral prior to your appointment, you must bring your referral form. Appointment concerning cosmetic and aesthetic concerns cannot generally be billed to insurance and referrals are not necessary.

For Aestheticians Appointments:

If you are a new patient please arrive 10 minutes before your first visit to fill out any paperwork.
