SkinCare Physicians' Dr. Kenneth Arndt was quoted in a recent Harvard Health Letter article entitled Do-it-yourself skin cancer checks. "About 50% of melanomas are identified by patients, and even more are discovered if the skin is examined with the help of a partner," shared Dr. Arndt. His advice is to simply ask your partner to "take 10 minutes a month to look for skin lesions that you haven't noted before or those that have changed in color, size or shape, and feel." He further suggested to "pay particular attention to spots that have grown and may display a variety of hues, such as tan, black, brown, pearly, or translucent; moles or pigmented spots that have changed in color, mass or contour; or sores that continually crust, bleed and itch."

SkinCare Physicians' Dr. Jeffrey Dover recently commented on the early intense pulsed light (IPL) devices and the recent engineering advances, in the March/April issue of MedEsthetics.

The IPL technology, used in photorejuvenation treatments to improve facial redness as well as visible signs of sun damage and aging, has been around for two decades. However, early IPL devices gained a negative reputation. “The early devices took a long time to warm up, and they were potentially hazardous, because they didn’t have a lot of safety features,” recalls Dr. Dover. “While early IPL devices worked for vascular facial lesions and lentigines and for treating photoaging in general, frankly we never used our device. The basic concept of intense pulsed light as an alternative to laser made sense, but the results were unpredictable.” Since then, IPL technology has made great strides in cooling and filtering. However, there is still a wide range in the quality of devices on the market. “It’s very unfair to lump all IPLs together,” cautions Dr. Dover, who uses the Palomar Icon and the Lumenis M22 at SkinCare Physicians. “These are two of the best IPLs on the market. They are very expensive and made by topnotch engineering companies. You get what you pay for when it comes to IPL.”

SkinCare Physicians' Dr. Jeffrey S. Dover has been named president of the American Society for Lasers in Medicine & Surgery (ASLMS). Dr. Dover will take office at Laser 2013, the ASLMS annual scientific conference held in Boston, Massachusetts, April 3-7, and will serve for one year. The ASLMS, the world’s largest multispecialty society of laser specialists, is helping to reshape how we diagnose, treat and prevent medical and aesthetic conditions using laser, light, and energy based technology.

According to the Melanoma Foundation of New England (MFNE) only 10 percent of melanomas occur on the neck and scalp. But those can be the most deadly because melanomas in those locations go undetected or are caught late. SkinCare Physicians' Dr. Robin Travers will be training salon professionals to identify cancerous moles on the scalp and neck in "The Skinny on Skin", a free education program launched by the MFNE. Drs Kenneth Arndt and Jeffrey Dover's research with picosecond (trillionthPicosecond technology transforms tattoo removal of a second!) laser devices for tattoo removal was highlighted in the recent Aesthetic Guide.

"Tattoo removal has graduated from the days of non-selective ablation using carbon dioxide and continuous wave lasers to selective photothermolysis using Q-switched lasers. The first breakthrough in tattoo treatments came 20 years ago, with the development of these Q-switched nanosecond lasers,” explained Jeffrey Dover who also conducted one of the clinical studies used in the PicoSure™ laser’s FDA approval process. “These billionth of a second pulse duration devices achieved scarless tattoo removal for the very first time.”

Many of our doctors at SkinCare Physicians attended the American Academy of Dermatology’s (AAD) 71st annual meeting this past weekend in Miami. 

Dr. Thomas Rohrer who is a member of the board of directors for the AAD, gave numerous presentations, demonstrated latest techniques and moderated various symposiums. Dr. Rohrer was actively involved in many fascinating sessions:
- Moderating the Resident Practice Management Symposium where he talked about hiring and maintaining exceptional staff in a dermatology office.
- Giving a live talk and demonstration on how to inject filler substances to attain optimal results, along with Dr. Michael Kaminer.
- Chairing and speaking in a session on surgical reconstruction.
- Presenting in a dermatologic surgery session about cosmetic tips and pearls where he explained how to improve aesthetic results with fillers and toxins.
- Giving a talk on pigmented lesion lasers (or removing brown spots and tattoos).  
During this biggest dermatology meeting of the year, Dr. Rohrer received a Presidential Citation for work he did on one of the committees restructuring the AAD communications.

Dr. Jeffrey Dover demonstrated the treatment of the aging face during “The State of Art of Aesthetic Dermatology" session along with Dr. Naz Saedi, who was trained as a fellow at our practice. As chair of the live injection session, Dr. Dover was responsible for selecting the best injectors in the Academy to treat live patients in front of an audience of 500 AAD members. They demonstrated a variety of injection techniques including neuromodulators and fillers.
In another live demonstration course, entitled Application of our Aesthetic Toolbox for the Treatment of Medical and Dermatologic Disorders, Dr. Dover treated a patient who had facial asymmetry after suffering from Bell's palsy. He also discussed ways to maintain a neuromodulator practice in the 2013 economy at the session Dermatologic Surgery: Cosmetic Tips and Pearls.

Dr. Michael Kaminer gave a lecture at the annual Residents Symposium on tools to evaluate the economic health of a practice. Dr. Kaminer also attended the MiraDry Advisory Board meeting where amazing new data were shared including near 100% efficacy for underarm sweat reduction.

SkinCare Physicians' Dr. Tom Rohrer was recently quoted in an article in the February issue of the Harvard Health Letter about the new official guidelines for Mohs surgery. Mohs surgery is a procedure that removes skin cancer while minimizi...ng damage to surrounding healthy tissue.
"There can be a slight bias toward using it for almost all nonmelanoma skin cancers, since Mohs surgery offers the highest cure rate of any surgical procedure for skin cancer," said Dr. Rohrer. These new guidelines will help dermatologists better select patients for the procedure and may also help patients save some money. "Down the road, some patients may find that insurance companies will not pay for them to have certain skin cancers treated with Mohs surgery, and they will have to opt for less sophisticated techniques", Dr. Rohrer added. Earlier this week, the JAMA Network for the Archives of Dermatology interviewed both Drs. Kenneth Arndt and Jeffrey Dover about their study evaluating the effectiveness of a novel Picosecond Alexandrite Laser for the treatment of tattoos. The objective of the study was to confirm that this new advance in technology would offer better healing and more clearing of tattoos with fewer treatments.
The results of the study showed that all participating subjects who completed the trial obtained greater than 75% clearing of their tattoos, and were either satisfied or very satisfied with the treatment!

Dr. Dover recently shared his view with the American Academy of Dermatology Association on how we, at SkinCare Physicians, are educating our medical dermatology patients about the breadth of cosmetic and anti-aging treatments we offer. We strive to educate our patients with tact and sensitivity. We display treatment brochures in every exam room as well as photo albums of patients before and after cosmetic procedures. When Dr. Dover sees that a patient has looked at them, he asks if he/she has any questions about cosmetic treatments and will eventually start the conversation about anti-aging procedures. He remarks that women “are much more attuned to their appearance, so the pickup is going to much higher with women than men. But, there is a whole new group of men who care very much about their appearance, and they’re very interested.” Finally, Dr. Dover stresses that timing is critical when starting the cosmetic conversation: “If the patient is in the office for a significant medical problem — for example, they think they have a melanoma, or a family member was diagnosed with melanoma — you just don’t bring it up, period. First there were facelifts, then Botox and other injectable treatments came along to help millions world-wide stop aging in its tracks. Last night, SkinCare Physicians' Dr. Michael Kaminer discussed a new, exciting cold therapy in an interview on WCVB: 'Frotox' could be next beauty, pain treatment.
The device is made by a start up company in California called Myoscience. It has been released in Europe, and clinical trials in the United States are about to begin.
The technology uses a novel cold therapy to temporarily eliminate nerve signals to muscles, which then weakens the muscles and gives a smoother appearance. For now, it is only used for the forehead and the wrinkles between the eyebrows.
The treatment is at least two years away from FDA approval, and not available in United States. However, SkinCare Physicians may be doing clinical trials with the device in the near future. Keep checking our Facebook page for more information.

SkinCare Physicians’ Dr. Dover commented on men's motivation to seek cosmetic treatments in an in-depth article published by the American Academy of Dermatology. The article, entitled The age of Brotox -  Noninvasive options lure more men to cosmetic treatment, appeared in the November issue of  Dermatology World.  

Dr. Dover remarked that among his male patients, he is seeing the biggest increase among the ones he characterized as “metrosexual” men: a “younger group, in general — men in their 30s, 40s, 50s — who care significantly about their appearance and want to be in touch with their feminine side.”
“If they’re heterosexual, they usually come in with their wife or girlfriend, or the suggestion has been made very strongly that they come in,” he said. “I actually prefer that they come in with that person because sometimes, if they come in alone, they say, I don’t know why I’m here, my wife told me I should come." In contrast, gay men and “metrosexual” men are more likely to come to him of their own volition. “And they’re actually a delight to take care of; they can be very particular, sometimes more particular than my female patients, which is interesting. They appreciate good results, so they appreciate our talent.”   

Looking at men's drive to use noninvasive body sculpting, Dr. Dover further stated that “sometimes they’re way overweight, but often they’re very fit but have a tiny bulge over their belts, or love handles, or gynecomastia. [...] Zeltiq [coolsculpting] works beautifully for localized areas of fat reduction — we get 22 percent fat layer reduction per treatment.”

Dr. Rohrer is featured in a video from the American Academy of Dermatology that explains how to check skin for cancer. He commented that current estimates show 20 percent of U.S. adults will be diagnosed with skin cancer in their lifetime, so it's important to be familiar with your skin, especially moles. "Checking your skin for skin cancer only requires your eyes and a mirror," Rohrer stated. "Examining your skin only takes a few minutes, but it could save your life." Watch the video.

No significant difference found between four resurfacing laser treatments
Drs. Arndt, Dover, Kaminer and Rohrer collaborated with other SkinCare Physicians and Boston colleagues in a comprehensive comparative trial evaluating the effects of four different fractional resurfacing lasers on twelve patients. This in depth study was just published in the journal Dermatologic Surgery, and the results have also been discussed in podium presentations at national meetings. Each patient was treated with each of the four lasers. All treatments resulted in improvement in photoaging and no significant difference was found between the lasers in outcomes, pain or histologic findings. This was the first and only such evaluation in the medical literature.

Arndt and Dover just returned from directing the annual "Controversies and Conversations in Cutaneous Laser and Cosmetic Surgery" advanced symposium, which was held at the Fairmont Banff Springs Hotel in Banff, Canada. Over 200 attendees traveled to this exciting and educational meeting to hear about state-of-the-art treatments and developments in this rapidly evolving area. Drs. Kaminer and Rohrer also participated in the program. "Controversies" , which has traditionally been held each summer for over the past two decades, is attended by physicians of many specialties, other health care providers and representatives from the medical laser and aesthetic industry, all of whom enthusiastically comment that this is the "best meeting of the year".

Would you have ever thought that fat could be removed without surgery?  Science fiction, right?  Well not anymore!  We are pleased to introduce CoolSculpting (by Zeltiq) and Liposonix to our patients, the first completely non-invasive techniques that remove fat.  SkinCare Physicians was one of a handful of centers in the world that was hand-picked to offer the first CoolSculpting and Liposonix treatments.  While there have been other companies and devices that have claimed to reduce fat without surgery, CoolSculpting and Liposonix are the first and only ones that actually do remove the fat, safely and permanently.  Several of our physicians have been involved with the research and development of CoolSculpting and Liposonix from the beginning, and we have proven scientific evidence that these techniques really do work.  The procedures are comfortable and take 1 to 3 hours. There is no need for anesthesia, while on occasion Liposonix patients receive injection of a mild pain medication (patients can still drive themselves home after either procedure) .During the procedure most patients read a book, listen to music, or watch television or a DVD.  The most common areas we treat are the upper and lower abdomen, love handles, hips, mid-back and bra line.  With some of our newer techniques, we are able to treat the arms, as well as both the inner and outer thighs.

CoolSculpting by Zeltiq

The way CoolSculpting works is through an ingenious method termed cryolipolysis.  Fat is more sensitive to cold than the rest of your skin. We selectively “supercool” the unwanted fat while the overlying skin is not damaged. Essentially, when fat gets very cold the body recognizes the fat as “damaged” and then removes it over time through natural processes.  It can take 3 to 6 months to see the final result, and in some patients a second or even a third procedure can be of benefit.  Research shows that as much as 25% of your unwanted fat layer can be removed in a single session. 

What is cryolipolysis? 

When fat is made very cold (just above the freezing point), fat cells become irreparably injured.  CoolSculpting is a novel technology that selectively makes fat cells cold, in turn causing permanent and effective fat cell loss.  Once the fat cells are treated, the body clears them through a natural process over a span of 3-6 months.
 
Is cryolipolysis dangerous? 

Extensive research on CoolSculpting has been performed, including right here at SkinCare Physicians.  Data reveals a superb safety record with CoolSculpting, and in multi-center research, there have not been any reports of adverse systemic effects from treatment.  Fat cells are gradually cleared from the body after CoolSculpting, so the liver and other vital organs are not impacted in any negative way. 

How much fat can you remove with CoolSculpting? 

A one hour session with CoolSculpting will treat an area about the size of your palm.  We can treat 2 to 3 palm-sized areas in any given session.  Research shows that CoolSculpting can eliminate up to 25% of the fat layer in each area we treat.  Additional treatments can remove more fat in any one area, so multiple treatments (to the same area) are cumulative. Liposonix uses ultrasound waves to gently eliminate treated fat cells.  Ultrasound technology has been used safely for decades in medicine, and Liposonix takes advantage of this proven method.  Once the fat cells have been exposed to the ultrasound energy, they are eliminated from your body via normal metabolism, never to return! 

Is Liposonix painful? 

Most patients feel only mild discomfort, and with some of our innovative techniques pain has been nearly eliminated.  For those patients who are sensitive, we use a mild injectable pain medication that does not cause drowsiness (Toradol).  Patients are able to drive themselves home, and there is virtually no discomfort once the procedure has been completed.
 
How much fat can be treated in one session with Liposonix? 

One of the advantages of Liposonix is the broad surface area that can be treated in a single session.  We can usually treat the abdomen and love handles in a single 60-90 minute session.  Thighs and arms take less than an hour for most patients. How long does it take to see results from 

CoolSculpting and Liposonix?
 
Most patients begin to see changes after a month or two.  It can take up to 6 months to see the final result.  Some patients elect to have a second treatment to eliminate as much fat as possible. 

Who are the best candidates for CoolSculpting and Liposonix?
 
CoolSculpting and Liposonix work best for patients who have a mild to moderate amount of extra fat that they would like to have removed.  Patients with small to medium sized specific areas of fat on the abdomen, bra-line, or love handles are ideal, and with recent advancements we can treat the arms and thighs!  Good skin tone is also helpful, but we are able to treat patients with stretch marks and a small amount of sagging skin.   Both procedures are useful in patients who would like to be a little smaller, but do not have enough fat to warrant having liposuction.  Most specifically, CoolSculpting and Liposonix are perfect for patients who want to remove fat but also would like to avoid having surgery. 

What are the most common areas that are treated? 

For now, we treat the love handles, hips, upper and lower abdomen, and bra line fat most frequently.  We have new technology that allows us to treat the outer and inner thighs, knees and arms. 

What is post treatment recovery like?
 
Most patients have absolutely zero recovery or down-time.  Immediately after the treatment the skin can feel sore or stiff, but this resolves quickly.  Redness can persist for a couple of days, and although unusual, some patients develop bruising in the treated area.  Infrequently, patients experience soreness in the area for a couple of weeks, and a slight change in sensation in the treated area for up to 8 weeks.

Botox for migraines? In an article just published in the Archives of Dermatology, research carried out by Drs. Arndt, Dover and colleagues from SkinCare Physicians and Harvard Medical School demonstrated that indeed it often can diminish or prevent certain types of migraine headaches. Patients who describe their symptoms as "crushing" or "vice-like" had a reduced frequency of headaches from 6.8 days per month to 0.7 days per month. Botox was used in the same fashion as it is administered for lines or expression or wrinkles. Continued use of this neuromodulator may bring striking relief to many who have long suffered with this difficult problem.

Dr. Jeffrey Dover has been awarded the prestigious Ellet H. Drake Lectureship Award from the American Society for Laser Medicine and Surgery, to be presented at their next annual meeting in April 2008. This award was established in 1995 to recognize a practicing physician who has contributed to innovative laser procedures and/or laser products for medicine. The award is given in honor of Ellet Drake, M.D., a co-founder of the Society. The awardee has to have contributed to innovative laser procedures, published in the medical literature, and have demonstrated excellence in teaching substantiating his laser activity during his career. This is an outstanding honor, and we are all very proud of our colleague!

Dr. Kaminer was the live in-studio guest for the popular Night Beat television show. The host was Portland Helmich, and the topic was hair loss for both men and women. Night Beat is well regarded as a forum for discussion of important topics in and educational, non-sensational, in depth format. The show aired on December 6, 2005.

One of the central topics discussed was the role of hair transplantation for women. It is now known that women can lose hair for genetic reasons just as men do, and fortunately hair transplantation is a successful treatment. the procedure takes about 4 to 5 hours, and can give women (and men) back the hair they have lost with time.

Other topics discussed included medical causes of hair loss, as well as non-surgical options in partnership with our colleagues at PK Walsh in Needham, MA.

The European Society for Cosmetic and Aesthetic Dermatology invited Dr. Kaminer to be their featured guest speaker in Paris. Dr. Kaminer spoke about the advances made with the Thermage technique, as well as the expanding number of options for non-invasive treatment of the face and neck. The Thermage skin tightening procedure has improved dramatically. With techniques pioneered at SCP, results have dramatically improved with Thermage, and pain has been virtually eliminated. Some of these new breakthroughs have not even been released to the general medical community! New areas are also being treated, including cellulite, as well as tightening of arms, thighs, and abdomens.

Kenneth A. Arndt, MD was awarded the prestigious Leon Goldman Memorial Award by the American Society for Laser Medicine and Surgery. The Leon Goldman Memorial Award was established in 1998 to recognize the many contributions in the areas of clinical laser research, laser patient care, and medical laser education of Leon Goldman, MD, the "Father of Laser Medicine". The award is given to a practicing who has demonstrated longitudinal excellence throughout their career in performing clinical laser research, providing high quality laser services to patients or educating others in medical laser applications. The individual must also share the characteristics of honesty, high ethical standards, and a dedication to patients that were possessed by its namesake.
