Insurance Referrals

To help your visit go smoothly and to avoid unexpected billing, it is essential that you verify that your insurance referral is in place and your insurance information details are accurate before your appointment. Most HMO insurance plans require an insurance referral number from your Primary Care Provider (PCP) before seeing a dermatologist. If a required insurance referral is not on file at our practice at the time of your visit, your insurance may deny coverage for the appointment.

Pre-visit insurance referral checklist

Before your visit, please make sure:

• You have seen your PCP first if required by your insurance:
  Many insurance plans require patients to be evaluated by their PCP before being referred to a specialist. If your plan requires this step, please schedule an appointment with your PCP so they can evaluate your concern and provide the necessary referral.
• Your PCP information is accurate with your insurance:
  Your insurance company must list the correct PCP in their system. If your PCP has recently changed, please update this with your insurance company prior to your visit.
• Your insurance information is active and correct in our records:
  Please make certain the insurance information we have on file is current and active. If your insurance has changed, notify our office before your appointment.
• Your referral has been sent by your PCP:
  If your insurance carrier requires an insurance referral, ask your PCP’s office to send it to our practice prior to your visit. To avoid billing issues, your insurance referral must be in place and active before the date of your appointment. If it has not been received, your insurance carrier may deny coverage for your visit, and you will be financially responsible.
