• You have symptoms of COVID-19 including: fever, chills, sore throat, loss of taste and/or smell, shortness of breath, dry cough, runny nose, abdominal pain, diarrhea (unless these symptoms are consistent with your annual seasonal allergies)
• You have been exposed to someone with a diagnosis of COVID-19 within the last 10 days
• You have a pending COVID-19 test
• You tested positive for COVID-19 within the past 14 days
